Player Registration & Volunteer Documents
2024 Player Registration Packet This packet contains the Registration Form, Medical Release, Parent Code of Conduct and Concussion form. Complete and print out all forms and MUST be turned in at a Document Collection Event during open Spring registration dates provided by the league with your Proofs of Residency.
Volunteers: All volunteers who will provide a "regular service to the league" and or "have repetitive access to, or contact with players or teams" MUST be cleared with a background check each calendar year: this includes managers, coaches, team parents, board members, and others. Contact the Safety Officer: [email protected] for a secure link to initiate the online background check. You'll also need to submit a copy of your drivers license to the Safety Officer.
2024 School Enrollment Form. Use this form if your residence DOES NOT reside within SCLL boundaries but your child's school DOES reside in SCLL boundaries: Dailard Elementary, St. Therese Academy, Hearst Elementary and Green Elementary.
SCLL Concussion Policy
Heads Up Concussion Online Course. Managers, Coaches & Board Members ONLY must complete the online course, print out certificate and turn in at document collection events or email to Coaches Coordinator: [email protected]
